For patients with severe functional mitral regurgitation and symptomatic heart failure (left ventricular ejection fraction between 20-50%) despite best medical therapy. The COAPT Risk Score is a simple risk prediction tool created by combining clinical and echocardiographic variables in patients with heart failure (HF) and moderate-to-severe or severe functional mitral regurgitation (FMR) who remain symptomatic despite maximally tolerated guideline directed medical therapy (GDMT). The COAPT Risk Score provides valuable prognostic information regarding the 2-year risk of death and/or heart failure hospitalization (HFH). The score is derived from the COAPT trial population of patients with HF and moderate-to-severe or severe FMR randomized to treatment with either transcatheter edge-to-edge repair (TEER) with MitraClip plus GDMT or GDMT alone. We can incorporate TEER in the risk score and provide updated 2-year risk of death and/or HFH which can serve as a valuable shared decision-making tool for patients and physicians as well as heart teams while deciding about TEER.
