A patient can review PAC facilities online and establish their preference. Case manager (from the hospital) would refer a patient to preferred LTC which can be dispositioned electronically. The nursing facility can internally manage live update bed availability at the facility. All medical records can be exchanged electronically. Nursing facility, hospital, patient & family receives live care transition notification.
