Measurements that you submit are automatically forwarded to your care provider so they can monitor your health remotely. The Heart for Health app offers the following Safe log in Each time you log in, we will send you a verification code via SMS, we do this to keep your account safe. Sending measurements to your care provider You can manually submit a measurement or use one of the connected devices to do so. Ask your care provider which devices are available to you. The measurements you submit are automatically forwarded to your care provider. You can also view your current and past measurements within the app. Notifications and reminders You will receive a message within the app when it’s time to perform a measurement.
