The Patient Safety Guide vs PRO Patient Companion Usage & Stats

Make the most out of your healthcare appointments, make your care safer, know where to go for help and what questions to ask, record your medication and personal health information make notes and have everything securely in one place with this easy-to-use FREE app developed with doctors, pharmacists and patients. It can also be used if you get help, or help look after, a family member or friend. You can use this app to • Identify where to go for help • Useful information and tips-plan what you want to discuss in a consultation with your doctor, nurse or pharmacist • Make notes after an appointment • Keep a diary to record notes and information • Prompts of questions to ask for planning before a consultation, using remote consultations, and going into and out of hospital, as well as patient safety information (eg what are my rights as a patient). This is important because: • It will help you get the most out of your appointment • Know what to do if something changes • Support you to identify different care and health management options • Can be shared with trusted people to support managing your health or others • Help to store your health information in one convenient place that you always have with you This app has been developed by the NIHR Greater Manchester Patient Safety Translational Research Centre. We have co-developed this guide with a lot of stakeholder involvement and engagement activities with patients, carers, members of the public, GPs and pharmacists. The guide aims to support patients and carers to be more actively involved in their patient safety in primary care which is a key national and international priority for patient safety and care. It is also available as a paper booklet, and a website (https://www.patientsafetyguide.org/). The Guide app is free to download and use. It does not provide medical advice, diagnosis or treatment. NOTE: This app integrates with the Health app so if you choose to manage certain heart conditions and have an Apple Watch or other device that creates heart rate data, you can send that data to your doctor through the app giving you full control over which data you send. PRO Patient Companion is a convenient and effective toolkit designed to help you manage your health by keeping a journal, tracking what symptoms you may be experiencing, your quality of life, what medications you are taking and your appointments with your clinician/healthcare team as well as learning more about how to manage your condition(s). Importantly, you will be able to share your health information (data) in a secure (HIPAA-compliant) manner that ensures your privacy and clearly communicates a more accurate picture of your “health story” to your care team. This enables your team to identify areas of concern and manage those more effectively leading to an improved health journey. Important and useful features of the app help you: • Manage multiple health conditions at once (selecting from a large list of chronic conditions including: ALS, Alzheimer's, Asthma, COPD, Depression, HCM, Parkinson's, AFib, High Cholesterol, Heart Failure, Psoriasis, CF, Diabetes, Eczema, GCA, Osteoporosis, Hypoparathyroidism, RA, NMOSD, Common Cancers and more) • Manage your medications and treatments, and set calendar reminders for taking medications • Record and track symptoms over time, keep a journal of daily ups and downs, moods, notes, activities and vitals. • Photo upload feature includes ability to markup photos or use a body map to create new image without a photo. • Track your Dr. Appointments and use the Checklist and Notes & Charts report to prepare for your next visit. • Report your quality of life and other scientifically validated medical measures by completing simple surveys.
