One Track Health is designed for people with chronic kidney disease (CKD) on dialysis and their care partners to easily view, track, & share blood test results, and stay up to date on the latest CKD-related news. The app is customizable so anyone tracking results can use it. Multiple profiles for multiple users can be added.
The top dashboard view allows you to see if you are within your set test reference ranges with a simple 2-color coded system. For people on dialysis and their care partners, there are 5 default tests included – Albumin, Calcium, Hemoglobin, Phosphorus and Potassium – with the standard test reference ranges & a Test Help Guide with educational information and references.
The color coding system makes it easy to view your results quickly. Green color indicates you are within your set test reference ranges and Red color means you are outside of your set test reference ranges. You can change the test reference ranges as you like via the test edit tool.

Key blood tests that are regularly monitored for people with chronic kidney disease (CKD) on dialysis (Albumin, Calcium, Hemoglobin, Phosphorus and Potassium) are included in this mobile and web app with standard recommended blood test result ranges for people with CKD on dialysis per NKF KDOQITM, KDIGO, UpToDate®, and consultant nephrologist. References used to determine the default test type ranges are included in The Blood Test Results Help Guide. We provide basic reference ranges for your educational purposes and recommend that you consult with your licensed healthcare provider/nephrologist for your specific reference ranges. The One Track Health mobile app is customizable, you can change the default test reference ranges via the test edit tool. See if you are within your clinician’s recommended ranges with a quick color view to determine if you are on or off track with your health goals.

Share your information with your care partner or physician as you like via the Care Partner Sharing Feature. You can allow your care partner to help you keep track of your results. Sharing a profile means that your care partner will be able to add, edit and delete both your test types and test results. Your care partner will not be able to delete your account or your profile. You will be notified of any changes your care partner makes ONLY via email.
