Get your care on the go with the Horizon NJ Health App. The Horizon NJ Health App is intended for use by NJ FamilyCare-Medicaid members enrolled in Horizon NJ Health. With the free Horizon NJ Health App, NJ FamilyCare-Medicaid members can: • View covered benefits, including medical, pharmacy, behavioral health, dental, vision and more • View, download or request a new member ID card • Change your PCP • Find a doctor, dentist or other health care provider • View your Care Manager, if you have one • Find community support and resources available to you • Get important reminders, such as when you need to renew your coverage NOTE: If you have Horizon Blue Cross Blue Shield of New Jersey coverage through your employer or directly, download the Horizon Blue app. Products are provided by Horizon NJ Health.
