"The MyPD patient mobile application provides peritoneal dialysis clinics and their patients with an interface to collect and view patient therapy exchange and vitals data. If your dialysis clinic is a participating clinic, then your clinic will be able to register you for MyPD and you will be able to use the app to perform the following key functionalities: • Protect user information by secure account login • Capture vitals data through Bluetooth as well as manual data entry • Capture Photos and share with care team • Capture and view CAPD exchange data • View APD exchange data • Automatically share details collected with your care team • View therapy trends and treatment history • View prescription details as prescribed by your care team • Get notified when there is a change to your prescription Refer to the user guide for the safe and proper use of MyPD.
