If a clinic visit doesn’t fit your schedule, use the app to launch a virtual telehealth appointment. Using this convenient service, you can talk to an experienced Memorial Care provider via secure video link and get a diagnosis and treatment plan without leaving your home. Check My Health This short questionnaire creates a snapshot of your current health and offers goals and strategies to improve your overall well-being. How It Works The first step is to create a profile within the Memorial Care app. This profile will include your name, birthdate and basic contact info.
