The Process Safety Incident Evaluation app will lets users evaluate a chemical incident against safety indicators. The PSIE app will have three main components: the Process Safety Incident (PSI) Evaluation Questionnaire, the Severity Weighting Questionnaire, and the Chemical and Mixtures Database. The PSIE app Questionnaire will walk a user, step-by-step, through the evaluation of an event leading to the determination of whether it meets Tier 1 or Tier 2 PSE Criteria. The Severity Weighting Questionnaire will help determine the severity rating of a confirmed Tier 1 event via a separate questionnaire. Last but not least, the app will enable users to save, e-mail or print the final report after answering either or both questionnaires. The chemical and mixtures database will provide a standard list of chemicals and mixtures. The app will also provide the user with a way to add, remove, or edit custom chemicals or mixtures to this database so long as the user can provide at a minimum number of fields.
