Part A Census Navigator™ uses the latest available data from the Centers for Medicare and Medicaid Services (CMS) to show you the flow of Medicare Part A patients from hospitals to skilled nursing facilities.
The app includes Part A admissions, star ratings and distances between hospitals and SNFs. You can search for any hospital or SNF and see the source of all Part A admissions.
