mChartsHealth vs. The Patient Safety Guide Utilizzo e statistiche
mCharts is a Personal Health Record (PHR) application that collects health records and clinical records from Apple Health and your other medical providers. It makes health and clinical records easy to understand and accessible to the right people at the right time. Your records are stored in highly secure, HIPAA-compliant systems in the cloud, so they are always available when needed, for routine care or emergency needs. Use this app with the mCharts Web App.
mChartsHealth retrieves all available medical/clinical information from Apple Health Records, which you have connected to your healthcare providers. The information is displayed in easy to understand views like the human body map. Additionally, mChartsHealth retrieves other Health and Clinical Category data which is used to help you set and track goals.
mChartsHealth displays health data from other providers, and health information you enter manually. It does not provide medical advice. Seek a doctor’s advice in addition to using this app and before making any medical decisions.

We have co-developed this guide with a lot of stakeholder involvement and engagement activities with patients, carers, members of the public, GPs and pharmacists. The guide aims to support patients and carers to be more actively involved in their patient safety in primary care which is a key national and international priority for patient safety and care. It is also available as a paper booklet, and a website (https://www.patientsafetyguide.org/).
The Guide app is free to download and use. It does not provide medical advice, diagnosis or treatment. Always seek the advice of your physician or other qualified health provider with any questions about your health, medical conditions or medications. This does not replace your medical record.
What people are saying
“I was concerned about stomach pains related to either colitis or menopause-the former might be serious. I read the guide to help me decide whether to talk to my consultant nurse at the hospital”
“I think it is a good structure. Particularly useful for anyone who may be intimidated or anxious when dealing with healthcare professionals”
“Increased [my] confidence in knowing the correct service was being accessed for my care”
“It really helped me to note down questions to take to my appointments and make notes on anything discussed so that I can keep track.”
“It’s very good”
“easy to follow and reminded you what to ask during telephone call with GP”
“I really liked the design of it. It was really easy to use, and I’ve actually got it here now so that’s jogging my memory as well a bit. But yeah, some apps can be really quite confusing to navigate. But it's so simple, you can just click on what exactly you need, make that form of those notes. And then I really liked I remember, the diary, where you can record your appointments as well”
“it made me think about other things I don’t normally think about when I go to the doctors, like, things to ask and things to look out for.”
“I think all that is very interesting and helpful.”
“I think it’s very useful”
