The application helps the care team manage their patients through their recovery remotely. Eligible patients are onboarded by a nurse on the smartphone app. Patients use the Patient App to add their daily check-ins of vitals and symptoms, if any. Nurses are immediately alerted of adverse information logged in, allowing quick followup with patients. Doctors generate digital care plans for the patient remotely.
